Please Note:
The website and document contain instructions for completing and submitting the survey form on the first page. It is important to remember the following in filling out the survey:

1. Do not alter the Excel format of the document.
2. Do not leave any boxes blank.  If the value of a certain line item is zero, insert a zero.  It's better to estimate a value rather than leave it blank.
3. Save the completed document to a safe location on your computer, and submit the survey directly to Profit Planning Group's secure mailbox (not to BRS/Profit Mastery), at surveys@profitplanninggroup.com.

*A participation cost of $200 (AMBA Members) and $275 (non-members) is being charged to customize the final reports so that each company participant may receive its own report to integrate into its business strategies and planning sessions. Once you submit your data, you will be billed by the AMBA. Results will not be provided until payment is received.

**Participation is limited to those AMBA members and industry mold builders that have been in business for at least one year as of their 2012 fiscal year-end (start date of January 1, 2012 or earlier required to participate in this study).
